Your First Visit

The first visit to the office requires a lot of administrative time. Please arrive 15 minutes earlier than your scheduled time to allow for this process. You can also save time by downloading, printing and completing the New Patient Form below. If you are transferring from another pediatric practice, we will need to review a copy of your child’s medical records. Please contact your current primary care physician and ask for a copy of your child’s medical records to be sent to our office or you may drop them off. It is important that we have at least your child’s vaccination history prior to your first scheduled visit.
